30 Minute Medicare/Private Standard Review Appointment - $100

45 Minute Medicare/Private Extended Review Appointment - $150

Note: An extended review appointment is recommended for more complex conditions or clients who require additional support.

Medicare

We are a registered provider for Medicare. You can access up to 5 Medicare rebates per year if you have a chronic medical condition and are referred by your GP under a Chronic Condition Management Plan (GPCCMP) (previously known as an EPC).

Eligible clients can currently receive a Medicare rebate of $63.40 per consultation.

To access the rebate, you will need:

• A valid referral from your GP

• A Chronic Condition Management Plan

• Available allied health visits remaining for the calendar year

Department of Veterans' Affairs (DVA)

Clarity Dietetics welcomes eligible DVA clients.

Dietetic services are fully funded by DVA for eligible Gold Card holders and eligible White Card holders when referred by their GP.

If you have a DVA referral, please bring your referral paperwork and DVA card details to your appointment.

Private Health Insurance

Many private health funds provide rebates for consultations with an Accredited Practising Dietitian (APD) through Extras Cover.

The amount you can claim will depend on:

• Your health fund

• Your level of cover

• Any annual limits that apply to your policy

We recommend contacting your private health insurer directly to confirm your eligibility and rebate amount prior to your appointment.

NDIS

Clarity Dietetics accepts:

• Self-managed NDIS participants

• Plan-managed NDIS participants

As Clarity Dietetics is not a registered NDIS provider we are currently unable to accept NDIA-managed participants.

Workers Compensation

Clarity Dietetics accepts approved Workers Compensation referrals.
